MAJOR DUTIES AND RESPONSIBILITIES:
1.Provides data that contribute to the assessment of a patient's or client's needs, the organization's capability to provide care or treatment, and the appropriate level of care.

2.Formulates, maintains, supports, monitors and modifies a patient-specific or client-specific plan for ongoing care management and treatment based on the patient's or client's assessment. Coordinates team functions to meet identified patient or client needs.

3.Incorporates growth and development and family dynamics in the development of an individualized plan of care considering customer sensitivity and the diversity of each patient or client.

4.Possesses expert nursing knowledge and skills. Utilizes skills and knowledge to provide nursing care to the patients, clients and families of a specialty care program.

5.Provides ongoing education to personnel, patients, clients or family through structured and unstructured interactions. Serves as liaison and advocate between patients, clients, families and team. Intricately involves patients, clients and families in goal setting and decision making.

6.Identifies and responds to variances in order to ensure patient's or client's continued progress toward health care goals. Initiates plans to minimize identified variances.

7.Demonstrates strong critical thinking skills by making appropriate nursing decisions. Rapidly prioritizes both planned and unplanned events, and proactively addresses actual and potential problems, exceeding patient or client expectations.

8.Provides for the exchange of appropriate care and clinical information when patients or clients are admitted, referred, transferred or discharged.

9.Demonstrates awareness of the value and relevance of research and evidence based practice in nursing. Begins to formulate research questions, which can be explored to validate or improve practice.

10.Networks actively with other health care professionals to promote smooth transitions and reduce boundaries in care delivery throughout the continuum of care.

11.Practice is recognized as clinically expert by peers.

12.Facilitates the identification and use of needed community resources.

13.Identifies need for educational program development, assists with development of programs, and evaluates outcomes.

14.Functions as a mentor and/or coach, formally or informally. Leads others in ways to improve quality, efficiency, productivity, and cost effectiveness of service.

15.Assists others and provides leadership in implementing the standards of the patient care and access process initiative (PCAPI) in care delivery.

16.Comply with policies and procedures protecting patient rights for informed consent and the handling of confidential information as defined by Cincinnati Children's mission and applicable laws and regulations. Review timeliness, accuracy, availability and security of information.

17.Perform procedures, collect data, analyze data and make decisions regarding patient care, including initial screening, assessment, diagnosis, care, reassessment and modification of care.

26.Perform other duties as assigned.

REQUIRED SKILLS: Proficient in characteristics of a Registered Nurse III. Excellent verbal, written and interpersonal communication skills. Capable of relating to diverse age and demographic backgrounds. Possess expert knowledge of the growth and development of pediatric patients and in the evaluation of deviations from the norm. Capable of utilizing research techniques in application to nursing practice and serving as a resource for educational opportunities in these areas. Demonstrate an independent work initiative, sound judgement, problem-solving skill, leadership ability, diplomacy and a professional demeanor. Possess motivation to promote continuing education, improved nursing practice and family-centered care.

EDUCATION\WORK EXPERIENCE: Registered Nurse and current license to practice nursing in the State of Ohio required; Kentucky and Indiana licenses may be required for specific positions. BSN required. Specialty certification may be required for some positions.
